Fall 2003 Online Course Design Workshop Application
October 13 – December 7, 2003
Thank you for your interest in participating in the fall 2003 Online Course Design Workshop. JesuitNET will use this application form to select faculty for the fall 2003 workshop. Please answer all of the following questions about your planned online course:
1. Your name, title, school, phone, fax, and email address
2. Proposed online course name, level (undergrad/grad/noncredit), and credits
3. Name, title and email of the library staff member available to assist you during the 8-week workshop (please check with your library dean or director.)
4. Name, title and email of the instructional technology specialist available to assist you (please check with your director of instructional technology or media services.)
5. Is your institution prepared to deliver the course entirely online, and using what course management system?
6. Indicate the first semester/quarter and year your course is planned to be offered.
Workshop hardware requirements include Broadband Internet access (LAN/DSL/Cable), SVGA or XGA monitor, and audio board with speakers or headphones. Workshop software requirements include Internet Explorer 5.5 or above (Netscape and AOL browsers are not recommended), Adobe Acrobat Reader 5.0 or above, Flash Player 6, and Quicktime 6 or above.
